Starting 1 November 2025, Australia will implement changes to its aged care system. The upcoming "Support at Home Program" will take the place of the existing Home Care Packages, unifying all in-home aged care services into a more straightforward system.

If you or a family member receive home-based physiotherapy or want to stay independent at home, here’s what these changes mean — and how Project Physio can help.

What is the Support at Home Program?

The Support at Home Program aims to simplify access to appropriate care and assistance for older Australians in their homes. It emphasises re-ablement and independence, supporting you in remaining active, mobile, and safe in your home environment.

Under this program, funding will be based on your assessed needs and goals. Services can include:

* Personal and domestic support

* Home modifications

* Nursing and clinical care

* Allied health services such as physiotherapy

The Role of Physiotherapy in Home Support

Physiotherapy continues to be a vital part of aged care under the new system. With the right physiotherapy support, you can:

  • Improve your balance and reduce the risk of falls

  • Recover strength after illness, injury or surgery

  • Manage long-term conditions like arthritis or Parkinson’s

  • Stay active and independent in daily life

At Project Physio, we’ll work with you and your Support at Home provider to make sure your therapy sessions are covered under your new plan — and that you’re getting the most out of your funding.

Why choose Project Physio

We’re a family-run local service, helping older adults live well at home and in the community. Because we’re local, you’ll always deal with real people who know your story — not a call centre.

Our skilled physiotherapists offer in-home services and have established robust connections with occupational therapists, equipment suppliers, and home-care coordinators throughout the region. As a result, we can assist in arranging everything from home safety equipment and mobility aids to coordinated treatment plans with your broader care team.

Here’s what you can expect from us:

  • Individualised care plans based on your goals

  • Home visits across the Sunshine Coast, Noosa Hinterland and Gympie regions

  • Collaboration with OTs, GPs, and equipment suppliers

  • Genuine, family-driven care focused on your independence

What you can do now

If you or your loved one currently receives aged care support, their Home Care Package will automatically transition to the new Support at Home system in November 2025.

It's the perfect time to:

  • Go over their care plan with their provider

  • Ensure physiotherapy is part of the future plan

  • Talk about what they wish to accomplish with additional support at home

  • Identify their physiotherapy goals

Our team can walk you through the process and help plan for a smooth transition.
